5 Star Home Care has compiled a list of organizations that you can contact for more information on various aspects of senior care. This list is not exhaustive or all-inclusive and is meant to serve you as a place to start if you are seeking additional information or resources.

ACLS Stroke Information & Resource Guide

Veteranaid.org
An informational resource for families wanting to learn more about the VA Aid and Attendance Benefit.

Veterans Care Coordination
Provides assistance to veterans and their families via free screening and application assistance. If a veteran or spouse appears eligible for the benefit, VCC will assist with care services during and after the application approval process and will continually assist with VA documentation requirements and expense monitoring to ensure continued eligibility. 5 Star works with VCC by providing an initial screening for veterans and their families, sending in professional referrals to VCC requesting that a representative reach out to the family, and following along in the approval process as needed.

Alzheimer's Association - Tennessee
Provides help and hope for those living with Alzheimer's disease and their families, seeking a cure to eliminate Alzheimer's disease through the advancement of research; to provide and enhance care and support for all affected; and to reduce the risk of dementia through the promotion of brain health. Tennessee Services for the Blind and Vision Impaired
The Department of Human Services' Division of Rehabilitation Services offers several programs to assist individuals who are blind or visually impaired. 800-628-7818

Caregiver Action Network
A leading family caregiver organization that provides education, peer support, and resources for family caregivers: 202-772-5050

Citizens Committee to Protect the Elderly
Provides guidance and support to families and residents of nursing homes and assisted living facilities and information about long-term care for the elderly: 757-518-8500

Tennessee Department for the Deaf, Deaf-Blind and Hard of Hearing
Outreach services for individuals who are deaf, hard of hearing, blind, or speech impaired: 800-628-7818

Elder Care Locator
A public service of the U.S. Administration on Aging, this nationwide service connects older Americans and their caregivers with information on senior services: 800-677-1116

Meals On Wheels
Nationally-based nutrition program providing meals to seniors who need them each day. Some programs serve meals at congregate locations like senior centers; some programs deliver meals directly to the homes of seniors whose mobility is limited, and many programs provide both services. The Nashville program number is 615-850-3910 or look up programs in your area on the website.

Medicaid
State-administered government assistance program available to eligible individuals and families with limited income and resources, covering a broad range of health care services: 800-342-3145

Medicare
Administered by the federal government, Medicare is health insurance for people 65 or older, people under 65 with certain disabilities, and people of any age with End-Stage Renal Disease: 800-633-4227
